At the bottom of this page is a link that will allow an applicant to download and print an application form to be completed and returned by email to the address on the form. Please read through the terms and conditions carefully.

Once you have sent in your application form it will be presented to the partnering practitioners. One or more of the practitioners will take your case and contact you to set up an appointment. Please make sure that the contact number you give is a mobile number, preferably with WhatsApp capability. If you have not been contacted for an appointment within 7 days of application please let us know by email.

All of our partnering practitioners have various skills and work with different treatment modalities. This means that treatment time can vary but once you are in the system we will make sure you get the best available treatment across all of the available modalities and protocols. Please be respectful of your practitioners availability as they are helping in the available time they have outside of their normal work.
